Northeast Florida Accountable Care, also known as NEFAC, is an Accountable Care Organization (ACO) formed to benefit the patient, provider and the community. We all share in its success. NEFAC is built on a model of coordinated care to ensure that patients, especially the chronically ill and the elderly, receive the right care at the right time, avoiding unnecessary duplication of services and prevention of medical errors. |
