Eddy SeniorCare is modeled after the established PACE national service model first developed as On Lok in San Francisco, CA, which has been operating across the country for more than two decades.

Eddy SeniorCare offers an interdisciplinary care team -- comprised of a physician, nurse, social worker, therapy professionals, pharmacist, day center staff, home care and transportation personnel -- all working together to manage each seniors'' medical conditions. Services include: day care, physician, nursing care, rehabilitation, medical specialties, social work, transportation, recreation, meals, personal care and in-home services.
