Fletcher Allen Health Care and Dartmouth Hitchcock Health have jointly created OneCare Vermont, a state-wide accountable care network comprising all but one of Vermont’s 14 hospitals, hundreds of primary care physicians and specialists, two federally qualified health centers and several rural health clinics, to coordinate the care of a portion of the more than 100,000 Vermont Medicare beneficiaries. |
