Three events in Seattle, Cleveland and Washington, D.C. to focus on patient care transitions challenges and solutions
The National Transitions of Care Coalition (NTOCC), an organization dedicated to addressing and improving patient care transitions in the U.S. health care system, will host a series of Transitions of Care (TOC) Summits this fall to raise awareness about the challenges of poor transitions and present solutions designed to improve transitions.
